The incidence of macroreentrant atrial tachycardias is increasing. Surface ECGs provide important information in the management of macroreentrant atrial tachycardias. Atrial flutter (AFL) represents macroreentrant atrial tachycardia on surface ECG. Catheter ablation of the cavotricuspid isthmus can terminate typical AFL with a high success rate and a low rate of complications. Atypical AFL is more difficult to eradicate. Electroanatomic mapping and a transseptal approach are sometimes needed in catheter ablation of atypical AFL. Recognizing the location of macroreentrant tachycardia from the 12-lead ECG can facilitate the mapping of reentrant circuit, guide ablation, and help in understanding the tachycardia mechanism.
